Our Registered Nurses will work with your primary physician to coordinate the delivery of any essential home health needs such as:

Case Management

Qualified nurses or therapists assess the physical, environmental, psychological, socioeconomic & eligibility under various third party programs. Rim 7 300x199 Skilled Nursing The clinicians develop a plan of care in collaboration with the patient, family, physician, & provider. Services are planned & coordinated with other disciplines & outside agencies. The plans of care are implemented as approved by the physician & payer. Clinical best practices are fostered through standards of care, training & evaluation processes.

Maternal/Child

Intermittent skilled nursing visits are provided to the pediatric population, including high-risk infants, young children, & teenagers with a wide range of diagnoses & special needs. In addition, home health services are offered to the pregnant woman to prepare her for delivery & early discharge &/or to manage high-risk prenatal conditions. Postpartum women & their infants are visited for a physical assessment & ongoing instruction in self-care & infant care.

Disease Management

A Registered Nurse designs a program collaboratively with the patient’s physician & other skilled nurses to help control the disease process & maximize wellness. Self care is promoted through educating the patient, family &/or caregiver in interventions to stabilize the disease process. The patient/family is taught monitoring & normalizing interventions. Signs & symptoms of complications & their prevention & nutritional interventions are also included in the disease management plan & education.

Wound

Rim of the World Home Health Inc. provides specialized care to patients with hard-to-heal wounds, skin disorders, incontinence, ostomy problems with the bladder & intestinal tract. Services include patient/family education, counseling of treatments & procedures, case management & access to community resources.

End of Life Care

Services are primarily directed at providing relief to a terminally-ill patient through symptom management & pain management, comfort care. The goal is not to cure, but to provide comfort & maintain the highest possible quality of life for as long as life remains. The focus is not on death, but on compassionate specialized care for the living.

Education

Education plans are designed within the plan of care by clinicians with specialty training specific to the patient disease process and rehabilitation. Rim 3 300x200 Skilled Nursing Examples may include maternal/child, infusion, diabetes, rehabilitation, or mental health. Patient, family &/or caregiver training is individualized to address barriers to learning & promote stability of disease process & reduce hospitalizations.

Infusion

Rim of the World Home Health Inc., in conjunction with a home infusion pharmacy of your choice, provides the highly trained & specialized skilled nurses to assist with the course of home infusion therapy. Nurses visit the home intermittently at a frequency tailored to medical needs. Technically advanced, cost-effective services are provided to support the patient &/or caregiver independence in the home as soon as possible. Various infusion therapies which may be infused in the home may include antibiotics, enteral nutrition, total parenteral nutrition, chemotherapy, etc. Services are coordinated with the pharmacy according to physician orders.
